Welcome to the practice of Nitin Chilakamarri, MD. Getting started is simple — request a visit online, and you'll complete your new-patient intake and consent forms securely through our patient portal. All of it happens in one place; nothing clinical is ever collected on this website.
1. Request a visit
Pick a time that works for you through our online scheduler. If you'd rather talk to someone, call 909.657.9535 and we'll help you directly.
2. Complete your intake in the portal
Once your visit is booked, your intake forms, insurance details, and consents are completed securely in the patient portal — the same place you'll message the office and view your records after your visit.
What to bring
- A photo ID.
- Your insurance card.
- A list of current medications, if you take any.
