This Privacy Policy explains how the practice of Nitin Chilakamarri, MD(“the practice,” “we,” “us”) handles information collected through this website. This website is a general marketing site. It is not a patient portal and is not intended to collect Protected Health Information (PHI).

Information we collect

When you use the contact form on this site, we collect the information you voluntarily provide: your name, email address, phone number, a general reason for contact, and any message you choose to send. The form is explicitly labelled as not secure for health information, and we ask that you not include medical details, symptoms, or other clinical information.

How we use it

• To respond to your request and schedule or coordinate an appointment.
• To verify insurance eligibility when you ask us to.
• To keep a record of our correspondence with you.

We do not sell your information, and we do not use it for advertising. This site does not run advertising or third-party tracking pixels.

Where clinical information goes

Any clinical information — new-patient intake, insurance documents, or health records — is handled through separate, HIPAA-covered systems (an intake form hosted by a vendor under a Business Associate Agreement, and our electronic health record's patient portal). Those systems, not this website, are governed by our Notice of Privacy Practices.

Data security

This site is served over HTTPS. Contact-form submissions are transmitted to a staff inbox and are not stored in a public database. Because no PHI is collected here, this site is not a HIPAA-covered data store.

Your choices

California residents have rights under the California Consumer Privacy Act (CCPA/CPRA), and health information is additionally protected under the California Confidentiality of Medical Information Act (CMIA).
